Choose Healthy Life Churches

Choose Healthy Life (CHL) improves health outcomes in the Black community through the trusted leadership of the Black Church, pairing faith and science to close gaps in care.

Member churches are funded congregations that host regular wellness events, where Health Navigators from the congregation provide free screenings and connect neighbors to care. Affiliate churches join CHL’s national network to carry health education, awareness, and action into their own communities.

Any church called to this work is welcome. To join the CHL family, apply to be an affiliate church.
